    Since Ethiopia's land reform act of 1975 and the nationalization of its major industrial and financial institutions, the government has organized society, has raised the level of literacy from 7% to 63%, and has conducted a 1st population and housing census. Now, in a 10-Year Perspective Plan, population policy is identified as a major issue, reflecting the country's concern over its present high rate of population growth--2.9%/year--and its infant mortality rate of 144/1000 live births, with life expectancy at only 46 years. Health care stratgy, including safe drinking water, is another top government priority, as is improving the status of women. Family planning services are offered, and Ethiopia holds that international assistance should reflect national sovereignty rather than being conditional to any particular family planning policy.

    Population crisis and desertification in the Sudano-Sahelian region.

    People living in the area just south of the Sahara Desert in Africa face their 3rd major drought since 1900. This drought brings about famine. Drought and famine are only manifestations of more profound problems: soil erosion and degradation. They diminish land productivity which aggravates the population's poverty. Yet soil erosion and degradation occur due to an expanding population. Continued pressures on the land and soil degradation results in desertification. The UN Environment Programme's Assessment of the Status and Trend of Desertification shows that between 1978-84 desertification spread. Expanding deserts now endanger 35% of the world's land and 20% of the population. In the thorn bush savanna zone, most people are subsistence farmers or herdsmen and rely on the soils, forests, and rangelands. Even though the mean population density in the Sahel is low, it is overpopulated since people concentrate in areas where water is available. These areas tend to be cities where near or total deforestation has already occurred. Between 1959-84, the population in the Sahel doubled so farmers have extended cultivation into marginal areas which are vulnerable to desertification. The livestock populations have also grown tremendously resulting in overgrazing and deforestation. People must cook their food which involves cutting down trees for fuelwood. Mismanagement of the land is the key cause for desertification, but the growing poor populations have no choice but to eke out an existence on increasingly marginal lands. Long fallow periods would allow the land to regain its fertility, but with the ever-increasing population this is almost impossible. Humans caused desertification. We can improve land use and farming methods to stop it.

    This discussion of the Expanded Program on Immunization (EPI) in the Americas covers training, the cold chain, the Pan American Health Organization's (PAHO) Revolving Fund for the purchase of vaccines and related supplies, evaluation, subregional meetings and setting of 1985 targets, progress to date and 1984-85 activities, and information dissemination. All countries in the Region of the Americas are committed to the implementation of the EPI as an essential strategy to achieve health for all by 2000. During 1983, over 2000 health workers were trained in program formulation, implementation, and evaluation through workshops held in Argentina, Brazil, Cuba, El Salvador, and Uruguay. From the time EPI training activities were launched in early 1979 through 3rd quarter 1984, it is estimated that at least 15,000 health workers have attended these workshops. Over 12,000 EPI modules have been distributed in the Region, either directly by the EPI or through the PAHO Textbooks Program. The Regional Focal Point for the EPI cold chain in Cali, Colombia, continues to provide testing services for the identification of suitable equipment for the storage and transport of vaccines. The evaluation of solar refrigeration equipment is being emphasized increasingly. PAHO's Revolving Fund for the purchase of vaccines and related supplies received strong support from the UN International Children's Emergency Fund (UNICEF), which contributed US $500,000, and the government of the US, which contributed $1,686,000 to the fund's capitalization. These contributions raise the capitalization level to US $4,531,112. Most countries are gearing their activities toward the increase of immunization coverage, particularly to the high-risk groups of children under 1 year of age and pregnant women. To evaluate these programs, PAHO has developed and tested a comprehensive multidisciplinary methodology for this purpose. Since November 1980, 18 countries have conducted comprehensive EPI evaluations. 6 countries also have had followup evaluations to assess the extent to which the recommendations from the 1st evaluation were implemented. At each subregional meeting, participants met in small discussion groups to review each other's work plans and discuss appropriate targets for the next 2 years. Immunization coverage has improved considerably in the Americas over the last several years. Figure 2 plots the incidence rates of polio, tetanus, diphtheria, whooping cough, and measles from 1970-83 in the 20 countries which make up the Latin American subregion. If all countries meet their 1985 targets, immunization coverages for DPT and polio will range from 60-100%, with most countries attaining coverages of over 80%. For measles, 1985 targets range from 50-95%, and from 70-99% for BCG. The main vehicle for dissemination of information is the "EPI Newsletter," which publishes information on program development and epidemiology of the EPI diseases.

    The United Nations Development Programme and women in development. Background brief.

    The UN Development Program (UNDP) began a special drive in the mid-1970s to ensure that women would enjoy greater benefits from its programs of technical cooperation. Efforts have increased steadily since 1975 when UNDP's Governing Council declared that "the integration of women in development should be a continuous consideration in the formulation, design, and implementation of UNDP projects and programs." They involve: promotion to create a greater awareness of women's needs and approaches which can meet them effectively; orientation and training to enhance skills in developing, implementing, and monitoring programs of benefit to women; improving the data base to provide better information on women's productive roles; programming to address women's concerns and generate self-sustaining activities, replicable nationally, regionally, and interregionally; and personnel action to increase the number of women professionals within UNDP. A number of projects supported by UNDP are directly benefiting women, especially those in rural and poor urban areas of developing countries. Among other things, these projects are helping to reduce women's workloads; addressing needs for clean water, health care, and education; providing training in basic skills; and helping to develop income-earning potentials. Examples are cited for the countries of Indonesia, Mali, Mexico, Yemen Arab Republic, Nepal, Rwanda, Honduras, Papua New Guinea, Liberia, Bolivia, and the Philippines.

    A fresh look at the threshold hypothesis of fertility change in ESCAP region

    The threshold hypothesis shares with transition theory the basic assumption that a decline in fertility is interrelated with a decline in mortality and change in the social, economic, and cultural conditions of the population. However, threshold theory fails to formulate a causal chain between fertility and the other variables and its application at the aggregate country level is limited by intracountry heterogeneity in cultural and social variables. Problematic is the fixing of the timing for a country of a decline in fertility to be inferred from the fact that some indicators of development have reached the threshold zone while others have not. This paper attempts to develope a combined index for socioeconomic development on the basis of data from 12 countries of the ESCAP region of South East Asia. Variables included were life expectancy at birth, infant mortality rate, adult female literacy, percentages of females economically active, GNP per capita, and percentage urban population. In 1970, 3 of the countries analyzed had a crude birth rate below 25, 6 countries had a rate between 25-40, and 3 had a rate above 40. The lowest value of the index recorded for countries of low fertility (crude birth rate below 25) and the highest value recorded for countries of high fertility (above 40) are taken as the threshold zones for the overall index. The number of countries in the threshold range increased from 5 in 1970 to 8 in 1975. With the increase in the index value, a reduction in the fertility level was noted. In contrast, where socioeconomic development was slow, fertility showed little change. Policy makers could use this system to assess which indicator could be pushed through to raise the overall index of development so as to effect a decline in fertility.

    The Evaluation Mission analyzes and assess the 2 United Nations Fund for Population Activities (UNFPA)-supported Women, Population and Development Projects and the role of women in 3 other UNFPA-assisted projects in Indonesia. The Mission concluded that the family planning and cooperative/income generation scheme as evolved in the 2 projects has contributed to increasing contraceptive acceptance and continuation and to a shift from the less reliable to the more reliable contraceptive methods. The projects have also assisted women and their families to expand their income generating activities, raise their incomes, and improve the family's standard of living. The Mission recommends that: 1) more diversified income producing activities be encouraged; 2) product outlets be identified and mapped and appropriate marketing strategies devised; 2) loan repayment schedules be carefully examined; 4) data collection, monitoring and evaluation be streamlined and strenghthened; and 5) the process of the entire rural cooperatives/income generation scheme be more comprehensively documented. In the 3 other projects, which are addressed to both men and women, the needs and concerns of women have not been adequately taken into account and/or the participation of women in all phases of the projects and their access to project benefits have not been equal to men. The Mission therefore recommends that special consideration be given to women's concerns in the design and formulation of all projects. The Mission ascertained that non-women specific projects tend to perpetuate existing discriminatory or unequal access to, and control of, resources by women unless specific consideration is accorded to them.

    Public health training of foreign physicians: trends over the last two decades.

    This presentation focuses on the changing role of US schools of public health over the past 60 years and covers predictions and trends of future changes. Foreign physician graduates of US schools of public health were not only responsible for founding the WHO, but have also served in positions such as director-general of WHO. Since World War II there has been an increase in foreign students trained in US schools of public health. Between 1965 and 1981 the number of foreign students increased from approximately 250 to about 700/year, and by 1983 the foreign student enrollment in US schools of public health had reached almost 1200. Most of the increase comes from heavily populated countries in Asia and in Africa. India was the country of origin for an average of 24 public health students in the US during 1967-68, but this number declined to 16 by 1977-78 and 1981. Nigeria significantly increased the number of trainees sent to the US from 5 students in 1967-68 to 54 in 1981. Although the total enrollment of foreign students has more than tripled since the 1960s, the % of foreign students in US schools of public health has dropped from over 20% in the early 1960s to about 13% in 1983. A review of all Johns Hopkins medical graduates shows that 75% of over 700 foreign medical graduate students live in their countries of origin, and only 14% live in the US. In general, the number of students from each country reflects that country's need. Assuming adequate levels of financing, US schools of public health should assist in the development of a sufficient number of schools of public health in their countries to meet those countries' needs for public health professionals.

    Public health training for physicians from abroad: current problems and a look at the future.

    At a time when there is a growing interdependency among nations with regard to trade, resources and security, there is an increasing provincialism in the US. In such a climate it is difficult to generate support for international programs. Involvement on the part of medical schools has waned almost to the point of nonparticipation in international medical affairs, largely because of constraints on training and residency programs. Academic health centers have not been supported as a matter of policy. Leadership in international health in other parts of the world, diminished involvement in international health, current priorities and programs and a future prospectus are discussed. The WHO seems an unlikely source for necessary leadership in helping define future directions for education or new strategies in preventive medicine and public health in the developing world. Institutions in Europe have deteriorated and participation and leadership from them are unlikely. Few people today are interested in clinical tropical medicine. Another reason for waning academic activity in international health relates to the paucity of interest on the part of foundations. An important initiative was the development about 5 or 6 years ago of the WHO Tropical Disease Research Program. It now has a budget of about US $25 million and has attracted additional money from the US and from other countries. A gamut of prospects has resulted including a maria vaccine, a leprosy vaccine, a new drug for malaria. In the developing countries, there is a much larger base of basic competence than existed only 10 or 20 years ago, but these health workers need support if health goals are to be attained. Schools of public health should be as much professional schools as schools of medicine, and the practice of public health should be engaged in. The US Centers for Disease Control (CDC), in its global Epidemic Intelligence Service (EIS) program in Thailand and in Indonesia has pioneered admirable new approaches in practical training. Provision must be made for sufficient faculty to permit both professional practice and education in any school that offers public health education. The US has a vital and unique role to play in public health and preventive medicine.

    During the past few years, 1 of the more exciting developments in vaccine production is the great improvement in the thermal stability of vaccines. Such improvements in vaccine stability, in combination with a carefully monitored distribution system (cold chain), have the potential for greatly increasing the coverage of an immunization program. The Expanded Programme on Immunization of the World Health Organization (WHO/EPI) has played a major role in aiding the development, adaptation, and field testing of equipment designed to meet the conditions encountered in the distribution of vaccines through the cold chain. An important innovation is the development of solar powered refrigerators for vaccine storage and ice making. In addition, WHO/EPI has attempted to identify the best methods for packing vaccine carriers and cold boxes. Since the rate of decline of vaccine potency is affected both by temperature and by age, it is important to know what temperature each vial of vaccine has been exposed to. Temperature monitoring devices that have been devised are discussed. Vaccination equipment (i.e., needles, syringes, and methods for sterilizing them) is essential to an immunization program. Alternatives to the syringe or needle for vaccine administration include the jel-injector and aerosol administration. Less expensive, more durable syringes are also being developed.

    The changing roles of women and men in the family and fertility regulation: some labour policy aspects

    There is growing evidence that labor policies, such as those advocated by the International Labor Organization (ILO), promote changes in familial roles and that these changes in turn have an impact on fertility. A conceptual model describing these linkages is offered and the degree to which the linkages hypothesized in the model are supported by research findings is indicated. The conceptual model specifies that: 1) as reliance on child labor declines, through the enactment of minimum age labor laws, the economic value of children declines, and parents adopt smaller family size ideals; 2) as security increases for the elderly, through the provision of social security and pension plans, the elderly become less dependent on their children, and the perceived need to produce enough children to ensure security in old age is diminished; and 3) as sexual equality in job training and employment and the availability of flexible work schedules increase, sexual equality in the domestic setting increases, and women begin to exert more control over their own fertility. ILO studies and many other studies provide considerable evidence in support of these hypothesized linkages; however, the direction or causal nature of some of the associations has not been established. Development levels, rural or urban residence, and a number of other factors also appear to influence many of these relationships. Overall, the growing body of evidence accords well with ILO programs and instruments which promote: 1) the enactment of minimum age work laws to reduce reliance on child labor, 2) the establishment of social security systems and pension plans to promote the economic independence of the elderly, 3) the promotion of sexual equality in training programs and employment; 4) the promotion of the idea of sexual equality in the domestic setting; and 5) the establishment of employment policies which do not unfairly discriminate against workers with family responsibilities.

    Demographic change in Europe and its health and social implications: an overview.

    This chapter presents an overview of recent demographic trends in Europe and discusses the implications of these trends for health and social services. The discussion is based on reports received from 15 of the 33 Member States of the European Region of the World Health Organization. The components of demographic change analyzed included population growth and structure, family formation, fertility, mortality, and population movement. Increases in the number and proportion of the elderly were noted and the traditional excess of births over deaths is expected to change in future years. Population aging is expected to continue to be a principal concern for the social services sector. The increasing emphasis on caring for rather than attempting to cure chronic illnesses among the aged suggests a need for more nursing homes and home-help services. Anticipation of future morbidity and mortality patterns implies a need to focus on specific risk groups, e.g. migrants, adult males, and those from lower socioeconomic groupings. With regard to fertility, adolescent sexual activity and the low use levels of contraception among teenagers comprise areas where greater service provision is necessary. In addition, there is a need for more vocational training for women, improved child care facilities, and full-time employment opportunities better suited to the needs of workers with dependent children. As a result of smaller families, increased divorce rates, the discrepancy between male and female survival, and greater regional mobility, markedly higher numbers of single individuals can be expected. Rapidly evolving changes in family formation, social norms, and underlying demographic trends will continue to alter European societies in the years ahead. The interrelationships between health and demographic phenomenon must continue to be probed to form a basis for future health and social planning.

    Women in national planning: false expectations.

    A strategy, developed by the Women's Programme of the Social Development Division of the Economic and Social Commission for Asia and the Pacific (ESCAP) to promote women's participation in the development process, is described. Although recommendations of international conferences invariably call for the inclusion of women in all levels of development planning, efforts to involve women in planning at the national level have met with little success. Even if women received sufficient training and education to qualify them as planners, their impact on development planning would be minimal due to deficiencies within the national planning process. Top planning units in most Asian and Pacific countries are composed of highly trained expatriots who lack an understanding of the needs of the population in general and of women in particular. The strategy developed by the Women's Programme is based on expanding the role of women in development planning at the local level and gradually sensitizing the planning hierarchy to women's needs and to women's abilities. This awareness building can be facilitated by developing links between government agencies and women's organizations. Application of this strategy revealed that it was much more difficult to build awareness among government officials and planners then to involve women in development at the local level. The planning process is constantly subject to personnel and policy changes because of changing political situations, and planners remain isolated from the public. At the community level, women's efforts to promote development are highly successful. Programs developed by women tend to benefit the entire community, and women's roles in these activities are highly visible. These successful efforts will contribute toward building an awareness of women's capacities to promote development. Conditions which are conducive to local level involvement of women include the political will to promote participation, the provision of appropriate training to prepare community members for participation, and the existence of an adequate infrastructure and sufficient resources to carry out programs.

    A perspective on long-term population growth.

    This paper presents some of the results of projections prepared by the World Bank in 1983 for all the world's countries. The projections (presented against a background of recent demographic trends as estimated by the United Nations) trace the approach of each individual country to a stationary state. Implications of the underlying fertility and mortality assumptions are shown mainly in terms of time trends of total population to the year 2100, annual rates of growth, and absolute annual increments. These indices are shown for the largest individual countries, for world regions, and for country groupings according to economic criteria. The detailed predictive performance of such projections is likely to be poor but the projections indicate orders of magnitude characterizing certain aggregate demographic phenomena whose occurrence is highly probable and set clearly interpretable reference points useful in discussing contemporary issues of policy.     Action by the United Nations to implement the recommendations of the World Population Conference, 1974: monitoring of population trends and policies.

    Pursuant to the recommendation of the World Population Plan of Action adopted in 1974, which was reaffirmed by the International Conference on Population in 1984, the United Nations has been undertaking a biennial review of population trends and policies. At the 22nd session of the Population Commission, held in January 1984, the Commission requested the Secretary-General to prepare an addendum to the concise report on monitoring of population trends and policies for the 23rd session, bearing in mind the relatively short time span since the preparation of the last such report. The purpose of the present document is to provide the Population Commission with such information to facilitate its deliberation on the agenda item. Analyses show that the gradual slow-down of global population growth is still holding with the present rate estimated at 1.65%/year, down from 2% during the 1960s. Declines have occurred in both the developed and the developing countries. Regional diversity of population trends have been so large that an overall global assessment seems almost irrelevant for policy consideration at national levels. The future population growth rate is expected to decline slower than it did in the past 15 years unless population policies change significantly. During the 1980-85 period the working age population (15-64 years) in the developing countries is estimated to have increased, on the average, at an annual rate of 2.8%, the elderly population (60 and over) at 3% and women in the reproductive ages (15-49 years) at 2.9%. The most urgent problem for many developing countries is perhaps the continuing very rapid increase of the working age population. The aging of the population, which bears significant policy implications, is among the most salient features of population change in the world, except for Africa. Fertility rates in most developed countries continue to fluctuate at low levels. No current data on developing country rates are available. An overall improvement in mortality in most countries is noted. A high rate of urban population growth in developing countries is a tremendous problem facing these countries. International migration, social and economic implications, demographic perceptions and governmental policies are summarized. National sovereignty, human rights, cultural values and peace are stressed as important factors in population policies. Women's status is discussed as playing a role in population change.

    In the last decade over 50 countries have strengthened laws or policies relating to fertility. Approximately 40 developing countries have issued explicit statements on population policy emphasizing the relationship to national development. In several countries constitutional amendments have been passed reflecting a more positive attitude toward family planning. High-level units, e.g. small technical units, interministerial councils and coordinating councils have been established to formulate policies or coordinate programs. Other actions relating to fertility include: increased resources for family planning programs, both in the public and in the private sector; elimination of restrictions on family planning information, services and supplies; special benefits for family planning acceptors or couples with small families, and measures to improve the status of women, which indirectly affects childbearing patterns. The recognition that policies, laws and programs to influence fertility are an integral part of efforts to promote social and economic development was reaffirmed at the International Conference on Population in Mexico City in 1984. 147 governments expressed their support for voluntary programs to help people control their fertility. Governments cite at least 4 reasons for increased attention to policies affecting fertility and family planning. Some of these are the desire to slow population growth to achieve national development objectives, concern for maternal and child health, support for the basic human right to determine family size, and equity in the provision of health services. In addition to the strongest laws and policies to lower fertility in Asia, legal changes are occurring in Latin America, Africa, and the Middle East. Family planning programs, laws on contraceptives and voluntary sterilization, compensation, incentives and disincentives, the legal status of women and fertility and policy-making and implementation are reviewed, as well as equal employment, education, political and civil rights and equality of women within marriage and the family.

    The United Nations' 2nd World Population Conference (Mexico City, 1984) called for greatly expanding funding for family planning assistance worldwide. The United Nations Fund for Population Activities (UNFPA), the conference's chief sponsor, will no doubt receive the largest portion of any assistance increase. UNFPA plays a critical role in population-related programs worldwide. The central debate on population policy should be over the extent and adequacy of the natural resources base and how countries can humanely and voluntarily change family size preferences. In countries like Singapore and South Korea, success has been achieved by combining social and economic incentives to discourage large families. Although couples in developing countries report wanting contraceptive service programs, they also want families of 4 to 6 children. So far UNFPA has been ineffective in changing the population situation. This overview of its activities reveals that UNFPA loses ultimate reponsibility for implementation of many of its own programs. UNFPA does not advocate a reduction in population growth within a single country, but rather helps couples have the number of children they desire. UNFPA's specific population and family programs are divided into functional areas: basic data collection, population change study, formulation and implementation of population policies, support for family planning/maternal child health programs and educational and communication programs. UNFPA stresses the importance of using contraceptives but not of achieving the small family norm. UNFPA's projects in some of the largest less developed nations are described, illustrating how the UN agency spends its assistance funds. From 1971 to 1982, the UNFPA spent almost US $230 million in the 10 largest less developed countries without any significant change in population growth. UNFPA program administrators are far from resolving the serious population problems facing developing countries and generally oblivious to new directions in which population policies should move. No progress will be made until UNFPA recognizes the need to approach the problem from a different perspective, working to change attitudes toward small families.

    Since the formulation of the World Population Plan of Action (WPPA) in Bucharest in 1974, about 80% of governments have endorsed family planning and fertility control. There has been a growing awareness by governments that population planning must be an integral part of general policy formulation. This article describes the issues of central concern to the 1984 International Population Conference in Mexico, highlighting those which result from new global developments over the past decade. Immigration, particularly by exiles and refugees from political persecution, are contributing much more to population instability than foreseen by the WPPA. Internal migration and massive population shifts from rural to urban areas are of increasing concern to governments in developing nations. In developed countries, there has been an emergence of anxiety over zero population growth. The role of privately sponsored programs for population control is much less prominent, as governments take more responsibility for formulating population policy. A report from a meeting of 90 such nongovernmental organizations held in 1983 was reluctantly accepted as an official document at the conference in Mexico. The Canadian Task Force on Population has identified 5 issues of special concern: status of women, the environment, aging, immigration, and family planning. The Task Force includes among its objectives the encouragement of a comprehensive population policy for Canada, focussing both on Canada's special concerns and on its place in the global community. For example, acid rain and improper soil conservation are threatening Canada's status as one of the few viable "bread baskets" for the world. The growing bulge in the population over age 65 will impose economic strain in the future. Sex education for adolescents in inadequate, with only 1/2 of Canadian schools addressing sex and sexuality in the curriculum.

    Familial roles and fertility.

    This paper presents a conceptual model indicating some of the established and hypothesized links between a number of labor laws and policies, in particular International Labor Organization (ILO) conventions, divisions of labor and resources by sex and age, familial roles and fertility. Brielfy outlined are the content and goals of some of the ILO conventions and programms that have a bearing on the conditions widely thought to be related to fertility decline. These include improved status of relatively deprive groups, such as women and children, and individual access to training, employment and incomes. These changes are viewed in terms of their potential impact on family relations, including changing parental roles and costs of bearing and raising children in view of the impact of diminishing child labor, and the increasing availability of social security benefits. Another aspect is sexual equality, in particular the impact of equality in the occupational sphere on equality in the domestic sphere and the consequent effects on reproduction. In addition, the impacts of social and spatial mobility are indicated and the potential effects on role conflicts, individualism and lower fertility. A thrust of the paper is to emphasize the critical intervening nature of changing familial roles, which have been neglected, both in labor reports and related activities as well as in the documentation and policy-making related to fertility. Micro-evidence from a variety of cultural contexts shows how changes and differences in allocations of tasks and resources and status benefits between kin, parents and offspring, wives and husbands are associated with changes and differences in fertility-related aspirations and patterns of regulation. Finally, the discussion serves to underline the pervasive and profound nature of the potential impacts of divisions of labor and employment policies on fertility levels, demonstrating that changes in familial roles and relations are central to this process of linkage. Thus, the need is made apparent for more knowledge and understanding of the dynamics of change in this area at the micro-level and in a variety of cultural areas, if government policies and programms are to achieve their specific goals with respect both to employment and demographic policies.

    In this speech the health miniter of Mozambique reviews his country's population situation during the decade since the 1st Population Conference at Bucharest. He emphasizes that in 1975, year of independence for Mozambique, the country had 10.5 million inhabitants, with a low average life expectancy (41.1 years), a high total fertility rate (6.6 children/woman) and a literacy level that was quite low with 93% of the population illiterate. In addition to poor socioeconomic conditions, the country has undergone political and natural disasters (e.g. war and drought) during the past 10 years. At the same time, the population's growth rate has continued to increase. Efforts to improve living standards include giving priority attention to education, health and housing. The adult illiteracy rate has fallen from 93% in 1975 to 72% in 1980. During the same period, a fourfold increase in the number of schools has been achieved. In the health field, primary health care and community participation efforts have succeeded at the implementation of immunization campaings and at the extension of health centers to rural areas. An important argument made here concerns peace as an essential requisite for development and the betterment of living standards. Recognized as serious factors facing developing countries in general, are imported inflation, paternalistic measures and policies, a deterioration of exchange relations, a worsening of taxes, and the balanne of trade deficits. It is in the context of these socioeconomic pressures that population issues become alarming. The government of Mozambique views population problems in the context of overall national development. A number of ongoing research projects for data collection and analysis are mentioned. Finally, the role of international agencies in promoting and financing development efforts is praised.

    The United Nations and the politics of population, 1960-74.

    One form of international authority proposed by David Mitrany was that of an advisory and coordinating one where both the performance of a task and the means for its accomplishment remain mainly under national control. Mitrany's theoretical framework and its organizational analogue within the UN and national political arenas account for the emergence of a new UN population policy to cope with the rapid global population growth between 1960 and 1974. The most prestigious outcome of this policy was the United Nations Fund for Population Activities (UNFPA), whose centralized contributions came primarily from the US, Japanese, Swedish, and some other west European governments. Its aim is to assist governments in the development of national family planning programs and in related demographic and family planning training and research programs. UNFPA grants went to UN-system agencies, governments, and private organizations. Recipients include India, Pakistan, Egypt, Malaysia, Kenya, Nigeria and Mexico. A mew ideology emerged to support the concept of an interventionist policy to lower the birth rate. That ideology include the responsibility of each government for its own population; an emphasis on social framework for parental choices about family size; and a legitimate role for international assistance. How the UNFPA came into existence is a political process involving government delegations and officials, UN Secretarist staff, and representatives of selected religious and population transnational organizations. It is also a Laswellian social process model of 7 decision-outcomes marking the significant population events and interactions underlying the creation of UNFPA. 6 UN resolutions and 2 decisions by the Secretary-General denominate these decision outcomes. 2 analytic approaches account for these decision outcomes--the Parsonian concept of organized levels (institutional, managerial, and technical) in conjunction with the Laswellian concepts of centralization/decentralization and concentration/decontration, and the concept of coalitions, (legislative and programming). This expanded UN population policy process reveals the interconnectedness of elites and groups in a global network centered at UFPA. (author's modified)

    A summary of developments in the population information field during the decade 1974-84 is presented. Progress has been made in improving population services that are available to world users. "Population Index" and direct access to computerized on-line services and POPLINE printouts are available in the US and 13 other countries through a cooperating network of institutions. POPLINE services are also available free of charge to requestors from developing countries. Regional Bibliographic efforts are DOCPAL for Latin America. PIDSA for Africa, ADOPT and EBIS/PROFILE. Much of the funding and support for population information activities comes from 4 major sources: 1) UN Fund for Population Activities (UNFPA): 2) US Agency for International Development (USAID); 3) International Development Research Centre (IRDC): and 4) the Government of Australia. There are important philosophical distinctions in the support provided by these sources. Duplication of effort is to be avoided. Many agencies need to develop an institutional memory. They are creating computerized data bases on funded projects. The creation of these data bases is a major priority for regional population information services that serve developing countries. Costs of developing these information services are prohibitive; however, it is important to see them in their proper perspective. Many governments are reluctant to commit funds for these activites. Common standards should be adopted for population information. Knowledge and use of available services should be increased. The importance os back-up services is apparent. Hard-copy reproductions of items in data bases should be included. This report is primarily descriptive rather than evaluative. However, given the increase in population distribution and changes in government attitudes over the importance of population matters, the main tasks for the next decade should be to build on these foundations; to insure effective and efficient use of services; to share experience and knowledge through POPIN and other networks; and to demonstrate to governments the valuable role of information programs in developing national population programs.

    Population on the world agenda, 1984: a view from Bucharest.

    Thise comments and remarks were fomulated in 1974 during a panel discussion which was part of the program for the Population Tribune, a nongovernmental meeting, organized in parallel with the 1st UN World Population Conference at Bucharest. The panelists discussed the ways in which they expected the deliberations of a similarly conceived international conference, taking place 10 years after Bucharest, would differ from those of the 1974 meeting. The author prefaces his comments by clarifying his own position: population change is nnot the determinant of economic and social development. 5 major differences between the future policy debates and those at Bucharest are identified, explored and critically judged. The next Conference's deliberations will be characterised by a greatly increased understanding and appreciation of what its topic is supposed to be, of what the population problem really is and of what population policy is about. The author argues that the present conference did not deal with these issues in a satisfactory fashion. He maintains that there has been a failure to identify the structure of the population problem: an inconsistency between collective and individual interest. The principle to be adopted by governments is to analyze their own situation, identify their problems and act according to their best interest. The principles are the same whether a country is developed or developing. A 2nd major difference will be an increased understanding and appreciation that population policies should be guided by a search for improvement and optimization. A 3rd important difference will be increased demographic sophistication of the participants, to overcome the mechanistic and naive interpretation of the development-fertility link. A 4th difference is the expectation that, by 1984, the economic sophistication in discussing problems of development will have been greatly increased, which will facilitate constructive discussions of economic-demographic interrelations. A final change expected for 1984 would manifest itself in a calmer yet more helpful stance of the developed countries with respect to the developing world in demographic matters. Ultimately, the solutions must be local, rather than global.

    The costing of primary health care: report of participation WHO consultation in Nazareth, Ethiopia.

    This meeting was sponsored by the World Health Organization (WHO) with Dr. Wayne S. Stinson participating at WHOs request. The objectives of the informal consultation were: 1) to strengthen national capabilities for undertaking the costing of preimary health care and for the utilization of results for development and management; 2) to exchange experiences on the costing of PHC in different countries; 3) to discuss methodologies used for data collection at the PHC center; and 4) to make recommendations for future work. This consultation is one in a series of costing and financing meetings held by WHO since 1970. The most recent meeting prior to 1983 was an interregional workshop on the cost and financing of primary health care, held in Geneva in December 1980. Papers distributed at that meeting (which have not yet been published) suggest a need for greater understanding of costing principles and technical refinement of methodologies. Judging by the papers presented at the Nazareth workshop, costing efforts have greatly improved since 1980. Representatives from the following countries participated in the Nazareth workshop: Argentina, Botswana, Columbia, Thiopia, Gambia, Kenya, Lesotho, Malawi, Sierra Leone, Sri Lanka, Swaziland, Tanzania, Thailand, Uganda, and Zambia. Some of these reported costing studies. This report consists of a narrative description of the meeting itself followed by a commentary on some of the issues raised. There is then a discussion of Arssi Province and Ethiopia as a whole based on a 1-day field trip. Finally recommendations are given regarding the United States Agency for International Development's (AID's) further PHC costing efforts.

    Concise report on the world population situation in 1983: conditions, trends, prospects, policies.

    The 3 parts of this report on world, regional, and international developments in the field of population, present a summary of levels, trends, and prospects in mortality, fertility, nuptiality, international migration, population growth, age structure, and urbanization; consider some important issues in the interrelationships between economic, social, and demographic variables, with special emphasis on the problems of food supply and employment; and deal with the policies and perceptions of governments on population matters. The 1st part of the report is based primarily on data compiled by the UN Population Division. The 2nd part is based on information provided by the Food and Agriculture Organization of the UN (FAO) and the International Labor Organization (ILO), as well as that compiled by the Population Division. The final part is based on information in the policy data bank maintained by the Population Division, including responses to the UN Fourth Population Inquiry among Governments. In 1975-80 the expectation of life at birth for the world was estimated at 57.2 years for both sexes combined. The corresponding figure for the developed and developing regions was 71.9 and 54.7 years, respectively. In 1975-80 the birthrate of the world was estimated at 28.9/1000 population and the gross reproduction rate was 1.91. These figures reflect considerable decline from the levels attained 25 years earlier: a crude birthrate of 38/1000 population and a gross reproduction rate of 2.44. World population grew from 2504 million in 1950 to 4453 million in 1983. Of the additional 1949 million people, 1645 million, or 84%, accrued to the less developed countries. The impact of population growth on economic development and social progress is not well understood. The governments of some developing countries still officially welcome a rapid rate of population growth. Many other governments see cause for concern in the need for the large increases in social expenditure, particularly for health and education, that accompany a young and growing population. Planners are concerned that the rapidly growing supply of labor, compounded by a trend toward rapid urbanization, may exceed that which the job market is likely to absorb. In the developed regions the prospect of a declining, or an aging, population is also cause for apprehension. There is a dearth of knowledge as to the impact of policies for altering the consequences of these trends. Many policies have been tried, in both developed and developing countries, to influence population growth and distribution, but the consequences of such policies have been difficult to assess. Frequently this problem arises because their primary objectives are not demographic in character.
