EQUILIBRES ET POPULATIONS. 2000 Jun-Jul; (59):4-5.A special UN session was held in New York during June 6-10, 2000, to evaluate the progress achieved since the Beijing Conference on Women. According to Françoise Gaspard, France’s representative to the UN Commission on Women’s Rights, negotiations at the special session were particularly difficult. It is always hard to create a satisfactory conference declaration when the rule of the day is consensus. A few countries always oppose such consensus. Latin American countries, however, abandoned their former position similar to that of Iran and the Vatican to instead adopt far more progressive stances upon reproductive rights. Progress is occurring slowly. While still not enough, the conference’s final statement marks a certain number of advances in the fight against violence, women’s role in decision-making, and education, with no steps back in the areas of contraception and abortion. The resulting declaration is therefore not regressive, even though it could have been stronger. It will hopefully serve as a reference statement which nongovernmental organizations will be able to cite when reminding countries of their obligations. Countries should get together to discuss the rising level of prostitution. The important roles of NGOs and French-country involvement were also recognized during the conference, as well as the priorities of education and funding.
Arlington, Virginia, Center for International Health Information, 1997 Dec. 16 p.This booklet presents highlights of 1995-97 activities of the US Agency for International Development's (USAID's) HIV/AIDS program. After a brief description of the current status of the pandemic, USAID's response, and its new strategy, the booklet provides a more in-depth examination of the HIV/AIDS pandemic, the highlights of USAID HIV/AIDS prevention activities during the past decade, and USAID's focus on prevention, which focuses on promoting safer sex behavior, increasing condom availability and use, and controlling sexually transmitted diseases (STDs). The next section of the booklet reviews USAID's proven interventions, such as behavior change communication and research, condom social marketing, and the development of services to prevent and treat STDs. An example is then given of how the three interventions were used successfully to stem transmission in Thailand. The booklet continues by explaining how USAID has targeted its response to developing countries (where it can have a significant impact on slowing the pandemic), youth, and women, and how peer educators and community outreach activities have been used to spread the prevention message. Next, the booklet discusses how USAID has expanded its partnerships with the World Health Organization's Global Programme on AIDS, with UNAIDS, and with Japan. The final section details the new USAID strategy for the future that will continue to focus on the three aspects of prevention and will also seek to mitigate the impact of HIV/AIDS on individuals and communities. The booklet also contains case studies of various USAID-funded projects.
PLANNED PARENTHOOD CHALLENGES. 1994; (2):42-4.INPPARES, the International Planned Parenthood Federation affiliate in Peru, has provided family planning and other services to the Peruvian population since 1976. The organization concentrates upon interventions targeted to women of low socioeconomic status. One of the group's most important strategies has been to distribute contraceptives at the community level in rural and peri-urban areas of the country through a network of centers managed by promoters. These promoters are virtually all female. The organization in 1993 supplied 812 distribution centers. Promoters and their supervisors have received training in contraception, basic data recording, community work, and related topics. INPPARES, however, suspected that the quality of the project would be improved if promoters and supervisors were trained about the role of women in the community and their rights and identity as women. The personnel would then be able to better understand the role of contraception and reproductive health in women's lives. To that end, INPPARES in 1992-93 developed a project in coordination with the Manuela Ramos Association, a Peruvian women's organization. A questionnaire was given to forty promoters on issues related to women's roles, values, attitudes, the place of women in society and the family, family planning, sexual relations, and decision making. Their responses pointed to a real need to provide promoters and supervisors with more information through workshops on women in Peruvian society, women's identity and roles, women's sexual rights, and the quality of care in service provision. Four pamphlets were drafted from a seminar of fifty supervisors from both organizations to be used in a series of twelve workshops for 256 promoters. Post-intervention evaluation of the original forty participants confirm the significant effectiveness of both subjects covered and materials used in achieving desired project goals. Four workshops were subsequently held in which project results were presented to 261 promoters. Promoters and supervisors are now using flipcharts and pamphlets in their training activities.
IPPF OPEN FILE. 1993 Feb; 1.In 1984, in Mexico City, the Reagan administration announced its policy prohibiting USAID from supporting any nongovernmental organization which used its own or US funds for any abortion-related activities. Even though this policy was intended to reduce the incidence of abortion, it had the opposite effect because the cut in funding left some areas of the developing world with no family planning services or information at all. Further, this policy resulted in a loss of $17 million (US) or 25% of the budget of the International Planned Parenthood Federation (IPPF). On January 22, 1993, US President Clinton reversed this policy. IPPF considered President Clinton's action to be a significant event for women's health, human rights, and global development. This reversal will provide family planning services to about 300 million couples who want to practice family planning but could not do so because they did not have access to it. Shortly after President Clinton's announcement, IPPF began writing a proposal to USAID for funds to restore programs that the Mexico City policy eliminated. IPPF hoped the reversal would spark international recognition of the need for safe access to abortion. Other actions President Clinton has taken to promote reproductive health are reversing the Reagan and Bush administrations' rule prohibiting abortion counseling at federally-funded clinics, requesting that the US Food and Drug Administration study the possible marketing of RU-486, removing the ban on abortion in military hospitals, approving regulations allowing fetal tissue research, and appointing an abortion rights advocate as Surgeon General. The Catholic Church opposed all of Clinton's abortion policies. However, many congregations, priests, and Vatican officials are dissatisfied with the Pope's anticontraception position.

The 1-year pregnancy rate is 0.2/100 users and the 5-year rate is 3.9/100 users. Contraindications of Norplant include abnormal bleeding, cardiovascular conditions, liver tumors, and breast cancer. The most frequent side effect is changes in bleeding patterns. A main concern of women's health advocates is that women are dependent on the medical establishment for insertion and removal of Norplant which affects the provider-client relationship. Family planning programs that do not recognize a woman's right to free choice of existing contraceptives and her right to have Norplant removed at any time may abuse Norplant. Health workers still do not know the long term effects of Norplant and Norplant's effect on the fetus in case of method failure or insertion while pregnant. Most acceptability studies occurred at university-based health clinics or at clinics in urban areas. The clinic environment may affect women's answers. These studies should occur in the community and home of users and nonusers. Another bias of these studies was clinic staff chose women who would tend to continue using Norplant. Thus subjects were not representative of the population. Researchers did not attempt to understand the women's perception of reproduction physiology and mode of action, the women's cost benefit analysis used to determine what method to use, or the consequences of menstruation changes. They also did not report on the information women received about contraceptive choices. The issue of abuse has arisen in Kansas where a state legislator proposed paying any mother on welfare US$500 if she uses Norplant. In California, a judge ordered a woman convicted of child abuse to use Norplant after release from jail and throughout her probation period.
