Abstract: Low-dose oral contraceptives can be used by a great number of women with IDDM or a previous gestational diabetes mellitus. Others hormonal contraceptives (progestogen-only preparations) are less convenient and the compliance is poor with an increasing risk of gynaecologic side effects. The safety of this contraceptives as regards vascular diabetic complications is not evaluated well. Nulliparous patients with progressive complications can use barrier methods of contraception after education. Macroprogestative contraception is a adequate choice for women within forties. For multiparous diabetic women, intra-uterine devices represent the first choice. (author's) French Abstract: Les jeunes femmes diabétiques nullipares insulinodépendantes et celles ayant des antécédents de diabète gestationnel peuvent, le plus souvent, bénéficier d'une contraception estro-progestative minidosée de 2' ou de 3' génération. Le recours à un autre mode de contraception hormonale (micro- ou macroprogestatifs) d'utilisation moins simple, d'observance plus hasardeuse et source d'inconfort gynécologique avec médicalisation accrue n'est pas justifié, d'autant qu'aucune n'a fait la preuve d'une plus grande innoccuité dans la prévention des complications vasculaires du diabète. La contraception locale après une information adéquate chez une jeune femme dont la personnalité permet l'observance représente, chez une nullipare avec multiples complications évolutives, une alternative possible. La contraception macroprogestative convient bien à la femme de la quarantaine. Une femme diabétique multipare doit en premier lieu bénéficier d'un dispositif intra-utérin.
